Thank you for visiting my appointment page. You can schedule your appointment with me here. Once your appointment has been booked, you will receive an intake package by email that must be completed and returned by email 24 hours before your appointment. If your paperwork is not received 24 hours prior to your appointment, your appointment will need to be rescheduled since I need to complete pre-work prior to your appointment. Due to COVID-19, all appointments are via phone at this time.
Requirements for the appointment:
1.) Most recent paycheck stub or other proof of income
2.) Most recent bank statement
3.) Completed Intake forms
Every appointment is via phone. Please make sure to call me on time since I normally have clients every hour on the hour. This will also help me to excuse myself from my previous client. If I do not receive a call from you I will call you to the phone you indicated on the paperwork. I look forward to speaking with you and helping you reach your goals.
